Ambassador Application

To become an official Home Run Dog Park Ambassador, you should have completed and passed the Rules Course, and the Etiquette Course. 

In doing so, you will have received a certificate.  You will need to full out the form below, and enter the dates from your certificates. 

A committee member will verify your completion dates, and send you a confirmation email with instructions on how to obtain your official Home Run Dog Park Ambassador polo shirt and other gear as it becomes available.
